To apply for this scholarship, you’ll need to prepare the following documents:

High school, college, or a combination depending on your year in school. Unofficial transcripts are acceptable. 

Must be completed on medical letterhead by your oncologist, primary care physician, or nurse. 

This can be requested via email through the application, or personally uploaded if you already have a copy.

This application also requires a written personal statement (maximum of 1000 words) and video essay upload .

Please cover (keep it simple):

1. Basics (≈20–30s): Your name, school you’re attending, graduation year, and major/program (or intended area if undecided).
2. What lights you up (≈60s): Share the passion that grounds you—music, art, writing/rap, volunteering, caring for others, building things, activism, faith, nature, etc. Why does it matter to you?
3. What you’re moving toward now (≈45–60s): A near-term aim connected to that passion (a project, community effort, creative goal, certification, or personal milestone).
4. How this scholarship helps right now (≈30–45s): The specific barrier it removes and what it enables (e.g., supplies, transportation, fees, reduced work hours, time/space to pursue your goal).
5. Close (≈10–15s): A hope you’re holding or one way you plan to pay it forward.

Optional (≤10s): A tiny sample tied to your passion (one lyric/line, a quick photo of a sketch, a brief clip). Your voice matters most—no performance required.

Tips: Record in a quiet, well-lit spot; landscape phone video is perfect. Keep any health details high-level—share only what you’re comfortable with. We pause reviews at 3:00 to keep it fair. If video is a barrier, submit audio-only or a 250–400 word statement covering the same points.
